RN Case Manager

RN Case ManagerImmediate need for a talented RN Case Manager. This is a 06+months contract opportunity with long-term potential and is located in U.S(Remote).Key Responsibilities:Conduct outreach to newly assigned and existing members to complete required assessments of member needs.Develop individualized care plans based on clinical, behavioral, and social needs, continuously reassessing as member needs evolve.Connect members to primary care providers, specialists, and community resources, facilitating scheduling and referrals.Provide education and support to ensure compliance with treatment plans, monitoring adherence to improve health outcomes.Identify and close HEDIS care gaps to enhance preventive care and chronic condition management.Maintain performance metrics by managing caseloads, completing assessments, and ensuring compliance with care plans.Key Requirements and Technology Experience:Key Skills: Experience in care management with a focus on chronic health conditions.Background in managed care and case management.Bachelor's degree in NursingCertification / License in RN Licensure [Required]2-4 years of experience in case management and/or managed care.Proficiency in Microsoft Office Suite and strong documentation skills.Excellent communication and engagement skills to build rapport and educate members.
