Start Date should be on or very quickly after the Order Start Date listed. All RTO should be listed on the cover page, must be less than 7 days. RTO will not be approved in the first week of assignment or holidays. All facilities the traveler is open to working in should be listed on the submission. If interested in "any" location, please include what states (I.E. Any AZ, Any CO, Any state, etc.) All shifts the traveler is open to working on should be listed on the submission. All departments the traveler is open to working must be checked on the cover page. All candidates submitted should be preclosed and ready to accept. SUBMISSION REQUIREMENTS

• 24 months minimum of current CVICU experience - REQUIRED
• Certifications must be American Heart Association OR client, active upon submission and INCLUDED IN SUBMISSION - REQUIRED
• No Nursys Hits - REQUIRED
• Banner will not approve RTO within the first week or holidays. Can not exceed 7 days.
• Flu vaccination - Banner declinations can be applied for (reach out for forms)

Designation as an Arizona Level I Trauma Center means Banner Thunderbird can provide total care for every aspect of injury, greatly reducing the need to transfer patients outside the area for life-saving treatment. Examples of injuries a Level I can treat include traumatic car crash injuries, gunshot victims, stab wounds and traumatic brain injuries.
