How you’ll make an impact in this role

• Champion Patient Care: Seamlessly execute physician and nursing orders with clinical precision, keeping patient safety and comfort at the forefront of everything you do.

• Be the Clinical Conductor: Serve as the primary coordinator across multidisciplinary teams, ensuring that physical therapists, physicians, and specialists are perfectly aligned for well-coordinated patient care.

• Monitor & Communicate: Vigilantly assess, record, and communicate changes in patient conditions, ensuring the entire care team has real-time insights.

• Advocate for the Next Steps: Collaboratively assess and coordinate comprehensive discharge planning needs, ensuring our patients and their families feel confident and supported as they transition home.

What minimum requirements you’ll need

Licensure / Certification / Registration:

• BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.
• Registered Nurse credentialed from the Wisconsin Board of Nursing obtained prior to hire date or job transfer date required. Licensure from the Wisconsin Board of Nursing OR current home state license if considered multi-state/Compact State.

Education:

• Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire.
