Position Summary

The Qualified Mental Health Professional (QMHP) – Crisis Stabilization provides direct support and case management services to children, adolescents, adults, and families experiencing mental health crises. This role is responsible for conducting assessments, coordinating services, maintaining compliance with documentation requirements, and ensuring clients receive timely, effective, and person-centered care. The QMHP serves as a liaison between clients, families, referral sources, and community partners while promoting positive outcomes and continuity of care.

Essential Duties and Responsibilities

• Conduct comprehensive initial assessments and evaluate client and family needs to determine appropriate services and supports.
• Develop, implement, and participate in individualized service plans within the required timeframe.
• Complete accurate and timely documentation, including daily progress notes using the B.I.R.P. format, monthly reports, and all required clinical records.
• Coordinate and facilitate referrals, resource linkage, and service connections for clients and families.
• Monitor service authorization periods, complete prior authorizations, conduct Medicaid eligibility verification, and communicate with referral sources regarding service continuation needs.
• Maintain regular communication with referral agencies, families, and community providers to ensure coordinated care and successful service delivery.
• Serve as a liaison between clients, families, referral sources, schools, healthcare providers, and community organizations.
• Ensure immediate client crises are addressed through effective intervention, stabilization, and support services.
• Assist in providing crisis intervention services as needed.
• Coordinate step-down planning and multidisciplinary staffing meetings prior to the completion of service periods to promote successful transitions.
• Conduct quality assurance reviews of client records to ensure accuracy, completeness, compliance, and program standards are maintained.
• Administer customer satisfaction surveys and assist with continuous quality improvement initiatives.
• Audit charts regularly and update documentation to maintain compliance with agency, state, and funding requirements.
• Maintain strict confidentiality and protect sensitive client information in accordance with HIPAA and organizational policies.
• Participate in regular supervision sessions, staff meetings, trainings, and professional development activities.
• Document supervision notes, meeting minutes, training records, and other administrative requirements.
• Maintain assigned caseload and ensure seamless transitions between assessment, counseling, and ongoing support services.
• Collaborate effectively with clinical staff, case managers, counselors, and community partners to achieve positive client outcomes.
• Provide administrative and program support across agency programs as needed.
• Perform additional duties and special projects as assigned by management.

Qualifications

• Bachelor's degree in Social Work, Psychology, Counseling, Human Services, or a related behavioral health field required.
• Qualified Mental Health Professional (QMHP) designation in accordance with Texas state requirements.
• Experience working with individuals and families experiencing mental health, behavioral health, or crisis-related challenges preferred.
• Knowledge of crisis intervention, case management, community resources, and behavioral health services.
• Strong documentation, organizational, and time management skills.
• Excellent interpersonal, communication, and problem-solving abilities.
• Ability to work independently while collaborating effectively within a multidisciplinary team.
• Proficiency with electronic health records and Microsoft Office applications.
