Community Case Manager - Flexible Schedule & Impact

Mental Health Association is seeking staff to provide intensive community-based Integrated Case Management Services (ICMS). Responsibilities include establishing therapeutic relationships, case management for up to 20 consumers, and coordinating with service providers for successful discharge. Qualified candidates have a behavioral health degree (or equivalent experience), are flexible to work evenings/weekends, and hold a valid driver’s license. #J-18808-Ljbffr Mental-Health-Association
