This hybrid role combines on-site visits with remote work from home, supported by a dedicated scribe team for efficient documentation. As a key member of our clinical team, you'll build strong relationships with the facility, maintain a patient-centered approach, and be a collaborative team player.Responsibilities:Provide high-quality preventative care to residents in skilled nursing facilities through on-site visits and remote work from homeConduct patient assessments, develop treatment plans, and provide education to patients and familiesCollaborate with facility staff to ensure seamless care transitions and comprehensive patient careMonitor patient progress, identify trends, and adjust care plans accordinglyMaintain accurate and timely documentation, supported by a dedicated scribe teamRequirements:Active NP or PA license (in the state of practice)1+ year of experience in:Geriatric careDocumentation reviewExcellent communication, collaboration, and teamwork skillsBenefits:Hybrid Role!
