You'll follow individualized care plans, administer treatments, and collaborate with the interdisciplinary team to ensure comfort and dignity for every patient.Key Responsibilities:Implement nursing care based on comprehensive assessments and care plansAccurately document patient services to reflect the plan of careEducate patients and families on diagnoses and self-care plansCoordinate hospice services with the interdisciplinary teamUse equipment and supplies effectively and efficientlyProvide pain and symptom management, evaluating patient responseParticipate in quality improvement and professional development activitiesQualifications:Valid California LVN licenseMinimum 1 year of hospice experienceExperience with Home Care Home Base (HCHB) preferredCurrent CPR certificationValid driver's license and insured, reliable vehicleAbout Seaport HospiceSeaport Hospice is part of the Pennant Group, a nationwide network with over 300 locations.
