AFED Nurse Practitioner, Per Diem

Position Overview The Division of Geriatrics seeks a per-diem Nurse Practitioner to support the Age-Friendly Geriatrics Emergency Department (AFED) consult service. This role involves providing consult services from Geriatrics to the ED by completing structured assessments, patient exam, and making recommendations to the ED and Primary Care Physicians for ongoing care or home management. The per diem position requires a minimum of 4 shifts per month, each 8‑10 hours, with onsite clinical coverage 7 days per week (approx 20% FTE per month, not to exceed 40% FTE). Responsibilities

• Conduct structured geriatric assessments and patient exams in the ED setting.
• Provide recommendations to ED and primary care physicians regarding ongoing care or home management.
• Coordinate with pharmacists, ED providers, geriatricians, social workers, community-based providers, and other interdisciplinary professionals to create optimal discharge or transition plans.
• Work collaboratively with geriatricians on scheduled shifts and with ED attendings and providers.

Required Qualifications Graduation from a California Board of Registered Nursing recognized Nurse Practitioner Program with a master’s or doctoral degree aligned with the population and clinical role. Two (2) years of clinical experience as a nurse practitioner in the geriatric specialty and/or setting. One (1) year of experience in a setting with the majority in geriatrics or as a Gerontology Nurse Practitioner. Preferred Qualifications Experience in nursing leadership and program management. Experience developing and implementing strategies to mitigate cognitive and functional decline in ED older adults. Two (2) years of specific geriatric experience in the past 5 years. One (1) year working in geriatrics as an NP. Experience precepting NP students. Ability to qualify as UCSF Health trusted interpreter in a non‑English second language. Experience using EPIC EHR. License/Certifications California Registered Nurse (RN) License without restriction. California Nurse Practitioner (NP) License without restriction. California Nurse Practitioner Furnishing Number. NP national board certification from an accepted certifying agency aligned with the patient population and clinical role (e.g., Adult Outpatient, A-GNP-C, ANP-C, FNP-C, GNP-C, etc.). Must be active and maintained throughout employment. California Driver License (if required for position). Drug Enforcement Administration (DEA) Registration with Schedules 2, 2N, 3, 3N, 4, and 5 registered to a California address. American Heart Association Basic Life Support (BLS) certification. Salary Information The final salary and offer components are subject to additional approvals based on UC policy. Placement within the salary range depends on experience and internal equity. For positions represented by a labor union, placement will be guided by the rules in the collective bargaining agreement.
