Family Outreach Coordinator page is loaded## Family Outreach Coordinatorlocations: Manchester, NHtime type: Full timeposted on: Posted Todayjob requisition id: JR2823**Job Description:**Easterseals NH is looking for a **Family Outreach Coordinator** in our Residential Educational Services program to provide case management services to youth and families in their communities for the purpose of reunification and also to support our Family Outreach Workers through mentoring, training and handling complex projects and cases as needed.Additional Responsibilities include:* Provide supervision, mentoring and training for Family Outreach Workers.* Manage complex projects and specific cases as needed.* Provide on-call services. Maintain a flexible schedule to accommodate family and program needs.* Assess existing and develop new family community resources.* Assist the youth and family in identifying strategies to support youth if their behavior is escalating.* Collaborate with Clinician, School Personnel, Nursing, Psychiatrist and other internal team members to make recommendations for treatment and to implement individual treatment plan.* Coach the youth, family member and Family Outreach Workers in identifying strengths and areas of need in order to develop and implement plans of care to be approved by the consulting prescribing practitioner.* Assist family members to take the lead in treatment planning by utilizing coaching strategies, training materials, and parent advocates.* Write and review contact notes, formal monthly reports and discharge summaries in collaboration with other professionals.* Maintain weekly (minimally) communications with family team members to ensure compliance with the treatment plan.* Provide oversight and support to Family Outreach Workers to implement trainings in youth-guided, family-driven practices, and Trauma Informed Care.* Provide services to family and support Family Outreach Workers in one or more family centered practices, such as family group conference facilitation, family safety planning facilitation, family time coaching, or family support.* Assist the family in finding resources for preventative and supportive services such as housing, transportation, community support groups, counseling and nutrition.* Design, develop and present family trainings based on the behavioral, emotional and social assessment of the family dynamic.* Provide in-home services and community connections with caregivers with focus on the implementation of the goals of the individual plan of care and to ensure compliance with the state's case plan.* Document and ensure compliance as it pertains to state certification/licensing standards, agency policies, other agency guidelines and contractual obligations such as data, daily and monthly progress reports, independent living requirements and independent living skills development.* Establish and maintain positive relationships with community members in order to connect the youth and family to their community such as employment opportunities, local school, social services, local community centers, social service agencies and leisure time activities.* Provide and/or coordinate family case management to include meetings, transportation, emergency services and behavior management.**Qualifications:*** Master's Degree in Human Services or related field (preferred) and two (2) years of experience, OR* Bachelor's Degree in Human Services or a related field and four (4) years of experience* Valid driver's license and reliable transportation**Compensation**: Salary is based on experience and qualifications.**Hours**: Full Time, 40 hours; Monday - Friday with On-Call Rotation.We offer a comprehensive benefits package for full-time, eligible employees, including:* Medical, Dental, Vision, Life & Disability* PTO begins accruing on your first day!* 10 Paid Holidays - includes a floating holiday of your choice* 403(b) employer match up to a maximum of 3%* Tuition reimbursement after one year of employment* Student loan repayment for qualifying degrees after one year of employment* Wellness programs: nutritional counseling services, reimbursement programs for weight loss & tobacco cessation**We are proud to be a recovery-friendly workplace and a NH Veteran-Friendly Business.****To learn more about this role**, please click on the blue **\"Apply Now\"**View email address on click.appcast.io.***EOE***Become part of a dynamic team dedicated to elevating all abilities, empowering lives, and fostering positive impact in the community.Join the Easterseals NH & VT family, where your passion meets purpose.Easterseals New Hampshire and Vermont elevates people of all abilities.
