RN Case Manager – Home Health We're hiring! Transitions At Home is currently seeking to hire a full‑time RN Case Manager in the Reedsburg area. Our growing home health agency is seeking a compassionate and experienced RN to coordinate and oversee patient care in the home setting, ensuring personalized, high‑quality outcomes for every client. What does a RN Case Manager do? Transitions At Home Registered Nurse Case Managers provide skilled nursing care to include assessments, medications, treatments, and coordinate services. They counsel the patient and family/significant others in their place of residence in accordance with the patient’s physician orders and plan of care, under the direction and supervision of the RN Clinical Manager and Administrator.
