Resources Coordinator

El Hogar Community Services, Inc

Summary: The Resource Coordinator is responsible to facilitate clients' access to service entitlements and deliver support services in alignment with the Psychosocial Rehabilitation Model and Strengths Model, specifically tailored for an adult population struggling with mental health, substance use, and justice involved. The Resource Coordinator will deliver recovery-oriented services, encompassing assessments, case management, skills training, and crisis intervention, among other vital components. The Resource Coordinator is responsible to identify and establish connections with community resources and community partners. Operating in diverse environments, including clients' homes, correctional facilities, psychiatric hospitals, or agencies, as needed.Essential Duties and Responsibilities: include the following. Other duties may be assigned;Client Transition and Community Engagement:Assist with client intakes, coordinating with discharging facilities and other community agencies.Conduct client visits in the field, including transporting clients to and from services.Facilitate the linkage of clients to community services, attending meetings and networking as needed.Support the transition of clients to outside facilities and services to ensure continuity of care.Client Assessment:Conduct comprehensive assessments of clients' needs, with a focus on identifying requirements related to benefits and housing support.Collaborate with treatment team and identified support persons to gather relevant information regarding clients' mental health and social needs.Benefits Coordination:Support & guide clients through the process of applying for and obtaining relevant benefits, including but not limited to General Assistance, Medicaid, Social Security Disability, and other financial assistance programs.Stay current on changes to benefit programs and policies, ensuring accurate and up-to-date information is provided to clients.Housing Support:Collaborate with client and treatment staff/identified support persons (if applicable) to develop comprehensive housing plans, taking into consideration safety, accessibility, and individual client requirements such as needs and challenges.Assist clients in navigating housing applications, subsidies, and support services to secure safe and stable housing.Identify appropriate housing resources and evaluate potential housing options to ensure they meet livability standards and align with clients' well-being.Conduct regular check-ins on clients' living arrangements to assess ongoing satisfaction and address any emerging issues promptly.Advocacy:Advocate on behalf of clients to ensure they receive fair and just treatment in securing benefits and housing.Collaborate with external agencies, social services, and community organizations to enhance and expand available resources for clients.Empower clients with information on self-advocacy, budgeting, and other skills essential for maintaining stable housing and accessing benefits.Documentation and Reporting:Maintain accurate and timely documentation and confidential records of interactions with clients within the Electronic Health Record system, per El Hogar and Sacramento County regulatory documentation policies & procedures.Generate reports on the status of client benefit and housing applications, providing regular updates to the clinical team.Collaboration:Able to work independently and with a multi-disciplinary team of professionals, including med providers, mental health clinicians, peers, care coordinators, and other members, to discuss/develop/facilitate integrated care plans for clients.Contribute insights and collaborate with team members to ensure comprehensive and integrated support for clients.Facilitate linkages and connections to community resources that enhance the well-being of individuals.Professional Development:Actively engage in training sessions, workshops, and conferences to enhance skills and knowledge.Adhere to and uphold the policies and procedures of El Hogar Community Services Inc.Ensure the protection and privacy of all Protected Health Information (PHI) by following all Agency policies and procedures.Supervisory Responsibilities:This position has no supervisory responsibilities.Qualifications:To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.
