SUMMARY: The Registered Nurse provides chronic hemodialysis care and treatment, develops nursing care plans involving assessment, planning, implementation, evaluation, training and education: communication with the physician and other members of the healthcare team to interpret, adjust, and complete patient care plans and assists in research studies. The Registered Nurse is responsible for assisting and supporting the unit management team in achieving quality, patient satisfaction, staff development, business management as appropriate, teamwork, culture and employee satisfaction. This position is also responsible for understanding, supporting, interpreting, and implementing the philosophy and policies of the facility. Provides safe, effective delivery of age specific patient care overseen by the Unit Medical Director in compliance with standards outlined in the facility procedure manual, as well as regulations set forth by the company, state and federal agencies.
ESSENTIAL DUTIES and/or RESPONSIBILITIES include the following:
- Demonstrates applicable knowledge of policies and procedures related to the Registered, Nurse/LPN/patient care tech for hemodialysis.
- Demonstrates ability to perform technical skills for the Registered Nurse including the patient's treatment, water treatment facility and emergency procedures, as determined by the competency assessment program.
- Share responsibilities of chart auditing with Charge RN.
- Delivers age specific care to assigned patients as prescribed, including all aspects of the hemodialysis procedure specifically trained to perform in accordance with state licensure.
- Demonstrates ability to work with any patient or staff, regardless of race, gender, disease process, life-style, religious or cultural beliefs, or treatment.
- Obtains, writes, transcribes and notes physician's orders as needed, and follows through appropriately.
- Initiates, monitors, and discontinues dialysis treatment according to established procedures. Assess patient's pre/post treatment and reports problems to the manager and Medical providers.
- Establishes priorities of care, both emergent and non emergent.
- Develops and assimilates theoretical and technical knowledge with observations, assessment, and communication skills to detect dialysis and non dialysis related problems.
- Recognizes and utilizes opportunities for patient and family education and training.
- Maintains a safe environment for self, patients and staff by utilizing proper body mechanics, and following universal infection control precautions and cleaning procedures under OSHA regulations.
- Dialysis isolation patients utilizing isolation precautions and prescribed procedures, if working in Walthill in Isolation.
- Assesses patient's pre/post treatment and reports problems.
- Performs hemodialysis access care according to established procedures.
- Administers and documents medication as authorized by a physician and as further authorized or limited by the State Board of Nursing Practice.
- Reports any significant information or change in patient condition to Unit Manager and Physician.
- Documents clearly, completely, and accurately all pertinent information on patient's manual or electronic record.
- Participates in the formation, update, and revision of nursing care plans including long and short term goals, according to established guidelines.
- Performs or assists in the performance of all emergency operational procedures, including the evacuation of patients in the event of a fire and/or disaster.
- Obtains blood samples and other specimens as ordered, prints labels as needed to label blood samples.
- Send information to Social Services Specialist to set up appointment and ride.
- Updates medication lists for patients, then prints and puts in chart.
- Monthly checks of meds and crash cart for expired meds.
- Care plans for patients following the list provided by the unit manager.
- Bi-monthly medication changes adjusted by lab results, prints new labels and follows all needed items to ensure all changes are done.
- Assists with and performs all procedures of equipment maintenance, safe operation, disinfection and inventory control according to departmental policy, manufacturer, recommendations and regulatory agency guidelines.
- Give supply list to Walthill Office Manager to be ordered.
- Assists in maintaining neat, orderly unit.
- Utilizes equipment, time and supplies economically.
- Maintains competency in operations, sanitizations, maintenance, and troubleshooting of all equipment used for dialysis, water treatment and special procedures.
- Actively participates with the unit manager and Charge Nurse in the care, routine cleaning, and use of equipment.
- Reports all equipment and supply problems appropriately and according to policy to repair personnel/biomedical support staff/Unit Manager.
- Records and reports all lab results as directed by the Unit Manager and Charge Nurse.
- Assumes administrative duties under the direction of the Unit Manager and Charge Nurse.
- Assists in orientation and training of new and existing staff.
- Acts as a resource person for staff members within the scope of practice.
- Assist in CQI projects with the healthcare team.
- Communicates any water, equipment, or facility problem quickly and appropriately following facility guidelines for reporting.
- Communicates effectively with physicians, supervisor, coworkers, patient and families.
- Maintains positive working relationship with Medical Director/Medical Staff.
- Actively participates in unit staff meetings and in-services.
- Assumes responsibility and utilizes opportunities for personal and professional growth.
- Promotes, supports, and adheres to all policies outlined in facility policies.
- Completes clinical training program to successfully master ESRD patient care for hemodialysis, management skills, leadership development and gain confidence in listening and communication skills.
- Acquires information on the latest developments of dialysis techniques by in-service classes.
- Seeks resources for direction, when necessary.
- Performs additional duties and is responsible for specific projects that are assigned by Unit Manager and Medical Director.
OTHER RESPONSIBILITIES:
- Responsible for maintaining a working knowledge of all hemodialysis and related ancillary and emergency equipment utilized in the facility such as water treatment equipment, computer, hemodialysis machines, sphygmomanometer, conductivity meter, glucometers, code cart, suction machine, oxygen administration equipment, and personal protective equipment.
QUALIFICATION REQUIREMENTS: To perform this job successfully, an individual must be able to perform each duty satisfactorily. EDUCATION and/or EXPERIENCE:
- Graduate from an accredited school of nursing. Bachelors in nursing or health related field preferred.
- Approximately on year nursing experience.
- Hemodialysis experience preferred, but will train.
- ICU/CCU experience preferred.
- Successful completion of training course in theory and practice of hemodialysis and peritoneal dialysis
PHYSICAL REQUIREMENTS:
- Moderately active work involving lifting and carrying: walking and standing for considerable lengths of time.
- Visual acuity to prepare and read written material accurately.
- Audio acuity to listen attentively and accurately.
- Ability to push, pull, mobilizes (with assistance of wheels, carts and other equipment) approximately 200 lbs.
- Meet the necessary requirements of Ishihara's Color Blindness test as a condition of employment.
WORK CONDITIONS:
- Exposure to potentially hazardous/infectious materials, observing and complying with the safety regulations/requirements of the facility and governmental agencies.
CERTIFICATES, LICENSES, and/or REGISTRATIONS:
- Valid State License
- Valid Driver's License
- CPR Certification; or must be completed within 6 months of employment
