Home Health RN Responsibilities Include:

• Perform comprehensive patient assessments and develop individualized plans of care

• Manage a designated patient caseload and coordinate care across the interdisciplinary team

• Evaluate patients' physical, emotional, social, and environmental needs

• Conduct initial assessments, routine visits, and ongoing re-evaluations

• Monitor patient progress and identify changes in condition

• Collaborate with physicians, therapists, caregivers, and other healthcare professionals to ensure quality outcomes

• Educate patients and families on disease management, medications, treatments, and self-care strategies

• Update and revise care plans based on patient needs and clinical findings

• Maintain accurate and timely documentation in accordance with regulatory and agency standards

• Promote patient safety and help prevent avoidable hospitalizations and emergency room visits

• Protect patient confidentiality and uphold HIPAA standards at all times

What You'll Bring

• Current, unrestricted RN license in the state of practice

• Minimum of one (1) year of nursing experience in a clinical care setting

• Strong assessment, critical thinking, and communication skills

• Ability to work independently while collaborating effectively with a multidisciplinary team

• Reliable transportation, valid driver's license, and current auto insurance

• Ability and willingness to travel throughout the assigned service area

• Ability to sit, stand, bend, lift, and move throughout the workday, including lifting up to 50-100 lbs. when necessary

Qualifications:

• Associate or bachelor's degree in nursing

• Home Health or Hospice experience preferred

• 1 Year Clinical RN experience required

• Experience with HomeCare HomeBase (HCHB)
