RN Case Manager - Hospice

Role The RN Clinical Nurse provides high-quality, safe, and patient-centered care as part of an interdisciplinary team. They perform the nursing process, advocate for patients and families, and ensure care is individualized and culturally sensitive. Responsibilities Coordinate and document care, promote collaboration through effective communication, support performance improvement initiatives, and participate in evidence-based practice and research activities. They also serve as preceptors or mentors to foster a competent workforce and assist in developing protocols to enhance patient outcomes. Qualifications & Skills Baccalaureate degree in nursing from an accredited institution, valid RN license (single or multi-state), Basic Life Support certification, and minimum 10 years of direct patient care experience. Must possess strong interpersonal, critical thinking, time management, and technological skills, with the ability to respond effectively in fast-paced environments.
