We are seeking an experienced and motivated Clinical Manager to join our growing home health team. This is an urgent hire for a hybrid leadership position that combines clinical oversight, staff support, and quality management. The ideal candidate is a strong clinical leader who can support field clinicians, ensure regulatory compliance, and help drive high-quality patient care. Responsibilities Monitor patient outcomes and clinical appropriateness Ensure care aligns with Medicare Conditions of Participation (CoPs) Supervise nurses, therapists, and home health aides Provide clinical guidance and decision support Assist with case conferences Address clinical concerns or patient status changes Assist with orientation and training of new cliniciansManagement of field preceptors Participate in QAPI (Quality Assurance Performance Improvement) Perform chart audits Ensure compliance with state, federal, and accreditation standards Assist with implementing process improvements Ensure documentation supports PDGM reimbursement Oversee case management Coordinate with physicians, hospitals, and interdisciplinary providers Address patient complaints Respond to incidents or safety concerns Assist with staff scheduling and productivity Assist leadership with clinical staffing needs Benefits Hybrid position (mix of field work, office time, and remote work from home) 401(k) Medical, dental, and vision insurance Paid Time Off (PTO) Supportive and collaborative work environment Qualifications Current RN license in good standing Home Health experience required AOSIS experience required Strong understanding of Medicare Conditions of Participation and PDGM Previous clinical leadership or management experience preferred Excellent communication and organizational skills #J-18808-Ljbffr
