The Community-Based Outpatient Clinic (CBOC) functions within a Patient Aligned Care Team Model. In collaboration with a dedicated team that includes a Primary Care Provider (PCP), Registered Nurse (RN), Licensed Vocational Nurse (LVN), and an administrator, the PCP manages a patient panel of up to 1,260 individuals. The PCP delivers primary care services in a collaborative environment, focusing on chronic disease management and preventive health measures. Common conditions treated include diabetes, hypertension, chronic obstructive pulmonary disease, degenerative arthritis, depression, congestive heart failure, and chronic pain.

The physician is responsible for evaluating, diagnosing, treating, and consulting with non-surgical adult patients across all age groups, handling a diverse range of diseases, injuries, and disorders that do not involve life-threatening complications. They assess, stabilize, and determine the appropriate course of action for patients with urgent medical conditions, adhering to medical staff policies regarding emergency and consultative services. The physician may also supervise mid-level providers such as nurse practitioners and work in partnership with various specialties (and subspecialties) both within and outside the organization to manage or co-manage patient health. Select mental health and subspecialty services are available on-site, while additional mental health, medical, and surgical subspecialty services can be accessed through consultations with the San Diego VA Health Care System or the community.

Schedule:

Monday to Friday, from 8:00 AM to 4:30 PM, excluding Federal Government holidays. There is no mandatory on-call duty.

Qualifications:

• U.S. Citizenship
• Unrestricted U.S. Medical License
• Completion of an ACGME or AOA accredited residency in Internal or Family Medicine
