Job Description

The direct support provider (DSP) staff is responsible for aiding program participants with

activities of daily living (ADLs), household management and community-based shopping or

appointments, working closely with family members and others to support the program participant to

progress toward more independent community living. The DSP is managed by the program manager.

Duties and Responsibilities:

• Assist residents with all activities of daily living, including showering, dressing, grooming,

toileting, mobility, feeding, taking medications, and Range of Motion (ROM) exercises, both in the

home and in the community.

• Prepare meals and provide light housekeeping.

• Assist residents with managing home, activities, and following goals in the annual service plan.

• Assist individuals with self-administering medications.

• Respond promptly to individuals' requests for assistance.

• Assist with teaching appropriate social and independent living skills.

• Encourage program participants to perform tasks as independently as possible, assisting them to

participate in all activities to the maximum extent possible.

• Accurately document provision of care as directed.

• Stay current with changes in care/support (e.g. medications, treatments, behavior plans, etc.).

• Alert program management in the event of an individual's change in status or behavior.

• Assist with weighing individuals and gathers and records other data as requested.

• Complete all documentation and incident reports within program timelines.

• Actively participates in resident, family, team, and staff meetings.

• Attend trainings as assigned, maintain certification of required trainings (CPR, First Aid and

Medication). Must successfully complete medication administration competency exam.

• Drives facility vehicles, as well as a personal vehicle.

Qualifications:

• A high school diploma or equivalent.

• Ability to understand, speak, read, and write fluent English, and sensitive to client's needs.

• Strongly prefer that a candidate will have a demonstrated understanding of and competence in

serving culturally diverse populations, can communicate effectively, use good judgment, high energy

level, function well in a team atmosphere, and perform basic computer functions.

• Must be able to perform each essential duty satisfactorily.

• Must have a valid driver's license and access to an operational and insured vehicle in addition

to a willingness to use it to transport residents.
