Contractor for Home Modification Class A, B or C Project Overview: Collaborative Home Modification Initiative for Aging in Place Charity Care Group, in partnership with the U.S. Department of Housing and Urban Development's Older Adults Home Modification Program (OAHMP), is launching a comprehensive initiative to enable low-income elderly homeowners in Norfolk, Virginia to age in place safely and comfortably. This Collaborative Project Aims To Implement low-cost, high-impact home modifications to reduce fall risks, improve safety, and increase accessibility for seniors aged 62 and older. Modifications may include installing grab bars, railings, lever-handled doorknobs and faucets, temporary ramps, and adaptive equipment. Utilize a person-centered approach, incorporating the expertise of licensed Occupational Therapists (OTs) to tailor modifications to each client's specific goals and needs. OTs will evaluate clients' functional abilities and home environments to optimize independence. Serve approximately 50 low-income elderly homeowners over a 3-year period, focusing on those unable to perform modifications themselves. Coordinate with local community organizations, healthcare providers, and senior services to identify and reach eligible participants. Provide education and resources on fall prevention, home safety, and aging in place strategies to program participants and the broader community. Leverage Charity Care Group's existing relationships and utilize Certified Aging-in-Place Specialists (CAPS) to maximize program impact and efficiency. Adhere to HUD's guidelines, including the $5,000 per unit cap for modifications, while ensuring comprehensive support for each participant. Contribute to the growing body of research demonstrating the positive impact of home modifications on fall prevention and disability reduction among community-dwelling elderly.
