Location: Fairfield, CA 94534, and its surrounding cities

Schedule: 10–12 hour shifts; Days (5:00 AM–10:00 PM) and Nights (6:00 PM–9:00 AM), including weekends

Pay: $75.00–$85.00/hour, based on qualifications, experience, and location

Sign-On Bonus: $5,000, paid after 90 days of service upon meeting eligibility criteria

We are seeking a Clinical Pharmacist for a full-time inpatient pharmacy position supporting the Inpatient Pharmacy at David Grant Medical Center, Travis Air Force Base, CA . This role offers the opportunity to work alongside physicians, nurses, specialty providers, and other healthcare professionals while providing comprehensive pharmaceutical care to hospitalized patients.

What You'll Do

Core Duties:

  • Provide comprehensive clinical pharmacy services to inpatient patients and multidisciplinary healthcare teams.
  • Assess patients' responses to drug therapy and develop medication therapy plans based on physician-established diagnoses.
  • Evaluate medication and medical histories to identify drug-related problems and recommend appropriate therapeutic interventions.
  • Manage patient-specific pharmacokinetic regimens, including vancomycin, anticoagulants, aminoglycosides, phenytoin, and other medications requiring therapeutic monitoring.
  • Order and evaluate laboratory tests necessary to assess medication therapy effects and clinical outcomes.
  • Initiate, modify, or discontinue medications for ongoing therapy of chronic disease states in cooperation with medical staff and established protocols.
  • Consult with physicians, nurses, dietitians, physical therapists, specialty practitioners, and other healthcare providers regarding patient treatment needs and pharmaceutical care options.
  • Participate in multidisciplinary rounds and antimicrobial stewardship activities to promote evidence-based patient care.
  • Document pharmacy interventions appropriately in the Electronic Health Record (EHR).
  • Assess metabolic needs and order appropriate therapeutic enteral or parenteral nutrition products in the inpatient setting.
  • Provide clinical direction and support to pharmacy personnel.
  • Review patient profiles prior to dispensing to closely monitor medication therapy.
  • Verify prescription directions, dosage, drug interactions, therapeutic duplications, allergy information, age-specific dosing, and other pertinent clinical information.
  • Ensure the accuracy of pharmaceutical products prepared or processed by technical staff.
  • Perform a full range of pharmacist procedures in accordance with established policies, procedures, and applicable regulations.
  • Administer prescription and non-prescription medications according to established agreements and Medical Treatment Facility (MTF) protocols.
  • Provide medication-related in-service training to clinical staff regarding new medications, appropriate medication use, therapeutic alternatives, and other pharmacy-related topics.
  • Facilitate medical staff Drug Utilization Reviews (DURs) as requested by the department head.
  • Document significant medication interactions and pharmacy interventions involving prescribers.
  • Document and report significant adverse medication events and patient safety concerns through appropriate reporting systems.
  • Conduct Adverse Drug Reaction (ADR) reporting in accordance with applicable accreditation and patient-safety standards.
  • Ensure compliance with USP <797> and USP <800> requirements, including appropriate protective equipment, beyond-use dating, and aseptic technique.
  • Maintain clean, organized, and safe pharmacy work areas in accordance with applicable regulations.
  • Ensure compliance with The Joint Commission (TJC) requirements throughout the facility.
  • Precept pharmacy students and residents in a non-supervisory capacity when applicable.
  • Assist with Outpatient Pharmacy operations as needed.
  • Demonstrate basic competency in Microsoft Word, Excel, and PowerPoint and maintain a typing speed of at least 40 words per minute.

What You'll Need — Mandatory Qualifications

Experience: Minimum 1 year of hospital-based pharmacy experience after graduation in unit-dose distribution and IV additives and/or completion of a hospital pharmacy residency is required.

Education: Graduate of a college or university accredited by the Accreditation Council for Pharmacy Education (ACPE) .

License: Current, active, and unrestricted Pharmacist license .

Life Support Certification: Current BLS and ACLS certifications required.

Security: Must be able to successfully pass a government background investigation and obtain the required government security clearance .
