Job Title: Home Health RN Profession: RN Home Health Specialty: RN Home Health Duration: 13 weeks Shift: Day Hours per Shift: 8:00 AM - 4:30 PM Experience: Minimum of 1 year Certified Home Care experience required. License: Active RN license required. Certifications: None specified. Must-Have: - 1 year Case Management experience. - Experience within a certified home health agency is required. - Ability to travel to assigned cases. - Weekend rotations every 6 weeks. Description: As a Home Health RN, you will be responsible for assessing patient conditions. You will implement care plans effectively. Providing ongoing education and support to patients and their families is essential. Patient care must align with organizational philosophy and community health practice standards. You will perform admission processes and case management responsibilities. This includes adhering to policies and standards of practice. Thorough and timely electronic documentation is necessary. You will engage in interdisciplinary communications to enhance patient care.
