Efficient Documentation: We utilize Kinnser (WellSky) for streamlined charting, What You’ll Do as a PT: Patient Care: Provide high-quality, one-on-one physical therapy as a field PT to geriatric patients in their homes. Care Planning: Implement effective plans of care to improve patient mobility and safety. Timely Documentation: Complete Evaluations, Re-Assessments, and OASIS documentation (Start of Care, Resumption of Care, and Discharge) with a clinical team that helps you get it right. Team Coordination: Collaborate with our clinical leadership team, dedicated office staff, and fellow therapy assistants to deliver outstanding patient care. What You Need: License: Active Florida Physical Therapist (PT) License. Certification: Current CPR/BLS Certification. Transportation: Valid Driver’s License and reliable transportation.
