Licensed Practical Nurse (LPN)

Hamilton Health Care Center is now under new management, and exciting changes are underway! We are building a strong, compassionate team dedicated to providing exceptional care to our residents while creating a positive, supportive workplace for our employees.

We are currently seeking Licensed Practical Nurses (LPNs) to join our growing nursing team.

We are looking for dependable and compassionate nurses who bring genuine empathy, patience, and professionalism to every shift. As a PRN LPN, you'll have the flexibility to pick up shifts while playing an important role in supporting our residents and nursing team when they need you most.

Job Responsibilities and Duties:

• Conduct assessments, follow protocol, report medical or behavioral changes, and document findings

• Perform admission duties: complete assessment, MAR, TAR, POS, review chart set-up, physician orders, and order medications

• Manage discharges: complete documentation, notify physician and guardian, and arrange transportation

• Provide wound care: follow protocols, irrigate, pack, and remove dressings

• Lead CNAs and CMTs, ensuring compliance with facility protocols and policies

• Respond effectively to emergency situations
