Nurse Practitioner

Carease Health - PA is seeking a dedicated and compassionate Nurse Practitioner to join our team of healthcare professionals in Butler County, PA. As a Nurse Practitioner, you will play a vital role in delivering high-quality preventative care to residents in skilled nursing facilities, working collaboratively with our team to reduce hospitalizations and improve patient outcomes.

About the Role:

As a hybrid role, you will split your time between on-site visits with patients and remote work from home, supported by a dedicated scribe team for efficient documentation. Your success in this role depends on building strong relationships with the facility, maintaining a patient-centered approach, and being a collaborative team player.

Responsibilities:

Provide high-quality, patient-centered care to residents in skilled nursing facilities
Conduct on-site visits and remote monitoring to detect abnormalities and prevent hospitalizations
Collaborate with our clinical team to develop and implement individualized care plans
Ensure seamless integration with the facility's processes and protocols
Demonstrate excellent communication, collaboration, and teamwork skills

Requirements:

Active Nurse Practitioner license (in the state of practice)
1+ year of experience in Geriatric care and documentation review
Excellent communication, collaboration, and teamwork skills
