As a Nurse Practitioner (NP) or Physician Assistant (PA) at our Senior Primary Care Center , you will collaborate with a diverse team that includes physicians, pharmacists, care coaches, and behavioral health specialists, all committed to delivering tailored, high-quality care.

Your workplace will be at our Central Stuart Clinic in Stuart, Florida, reporting directly to the Associate Medical Director.

• Deliver comprehensive care to seniors in accordance with clinical standards.
• Engage daily with your care team to ensure coordinated and high-quality service.
• Oversee referrals, coordinate with hospitals/SNF, and manage DME and home health services.
• Accurately document care with assistance from quality-focused coders.
• Participate in on-call rotations and contribute to growth initiatives at your center.
• Demonstrate a commitment to providing value-based care.

Required:

• Master’s Degree in Nursing or completion of a PA program with board certification.
• Active, unrestricted NP or PA license in the state where you will practice.
• Selected candidates will need to undergo TB screening.

Preferred:

• Experience in value-based care, primary care, or Medicare Advantage environments.
• Possession of an active DEA license and Medicare/Medicaid provider numbers, or the ability to obtain them.
• Familiarity with HEDIS quality indicators and Medicare regulations.

Language Proficiency Testing: Any associate who communicates with a member in a language other than English must complete a language proficiency assessment, provided by an external vendor, to ensure competency. Applicants will be required to take the Interagency Language Rating (ILR) test as mandated by the Federal Government.

The salary range provided below reflects a good faith estimate of the starting base pay for full-time (40 hours per week) employment at the time of posting. Actual pay may vary based on geographic location and individual qualifications, including skills, experience, education, and certifications.

$123,800 - $170,300 per year

This position is eligible for a bonus incentive plan based on individual and/or company performance.
