Community-based position requiring travel to client homes and community locations.

Valid NYS driver’s license and reliable transportation required.

What You’ll Do

• Provide Health Home care management and care coordination• Conduct community and home visits• Develop person-centered care plans• Connect members with medical, behavioral health, housing, and community resources• Coordinate with providers and support systems• Complete timely documentation and maintain compliance standards

What We’re Looking For

• Bachelor’s degree in human services or related field preferred• Experience in case management, behavioral health, social services, or community support preferred• Strong communication and organizational skills• Dependable, professional, and compassionate approach
