Case Management Role

Parkland Farmington 1101 W Liberty St Farmington, Missouri 63640 This position will be onsite! Seeking candidates with Case Management experience.

Full-time; days Monday - Friday Weekend On-Call as Required by Department Benefit Eligible

Additional Preferred Requirements RN experience in inpatient case management

Overview As part of BJC HealthCare, Parkland Health Center offers a unique combination of world-class medicine along with the deep local roots and rich history of its three predecessors Bonne Terre Hospital (opened in 1911), Mineral Area Regional Medical Center (1952), and Farmington Community Hospital (1969). Parkland Health Center has two locations in St. Francois County, Missouri a full-service acute care hospital in Farmington as well as a rural emergency hospital in Bonne Terre. Parkland Health Center provides quality primary and secondary level inpatient and outpatient services to the people of St. Francois County and surrounding areas in southeastern Missouri.

In addition, Parklands Farmington campus offers a full continuum of inpatient and medical-surgical and intensive care, obstetrics and emergency services, along with a wide variety of outpatient services including oncology services, an infusion center, a wound care clinic, and the Huckstep Heart and Lung Center. Bonne Terre is a Rural Emergency Hospital providing emergency services and outpatient services including medical observation, lab, radiology, and sleep study.

The Care Coordination Department provides support for the accurate and timely processing of patient care at PHC. This care includes finding the highest quality discharge planning possibly within our local community.

Preferred Qualifications Role Purpose Coordinates, negotiates, procures and manages the care of patients to facilitate achievement of quality cost-effective outcomes. Works collaboratively with physicians and interdisciplinary staff, internal and external to the organization, to identify and resolve issues. Performs and provides consultation regarding Utilization Management and Performance Improvement programs in order to obtain optimum value for the patient and the reimbursement source.

Responsibilities Assesses, coordinates, and evaluates each patient's plan of care. Establishes, continuously evaluates, and documents an individualized discharge plan. Ensures clinical documentation and cost-effective use of hospital resources.

Minimum Requirements Education Nursing Diploma/Associate's - Nursing Experience <2 years Supervisor Experience No Experience Licenses & Certifications RN

Preferred Requirements Education Bachelor's Degree - Nursing Experience 2-5 years
