Associate Patient Care Coordinator

Role: Patient Logistics Coordinator RN for Psych Intake Responsibilities Coordinate psychiatric patient placement, admissions, and transfers. Conduct clinical intake assessments to determine patient needs. Collaborate with physicians, nurses, and facility staff to facilitate safe and timely care transitions. Monitor bed availability and verify clinical appropriateness for placements. Communicate effectively with families and community partners regarding patient status. Document all activities accurately in EMR systems. Ensure compliance with regulatory and safety standards. Support continuous improvement of patient flow within a compassionate, patient-centered environment. Qualifications & Skills Registered Nurse (RN) licensure. Strong clinical assessment and coordination skills. Excellent communication and collaboration abilities. Knowledge of EMR documentation and healthcare regulations. Ability to work effectively in a fast-paced, patient-focused setting. #J-18808-Ljbffr
