Coordinate concurrent utilization management activities, serving as the main link between the patient, care team, organization, and payors during the hospital stay. Lead, educate, and collaborate with an interdisciplinary team during daily rounds, care conferences, and care planning. Communicate effectively in writing, verbally, and via electronic systems and applications used in patient care. Partner with community organizations to secure durable medical equipment, health insurance authorizations, and transportation. Participate in length of stay management for the inpatient population. Apply knowledge of hospital reimbursement methods and quality measures relevant to care management. Requirements Active Registered Nurse license; Associate or Bachelor degree‑prepared. 3–5 years of clinical experience in medical/surgical nursing, home care, hospital case management, and/or prior utilization review experience. Utilization review and discharge planning experience. Certification by a nationally recognized case management or utilization review organization preferred; CCM or ACM preferred.
