At Saugus Rehab and Nursing, we are passionate about providing exceptional care and support to our residents, and we are seeking a Responsibilities Assess the social and emotional needs of residents and develop individualized care plans to meet those needs Provide counseling, guidance, and support to residents and their families to address concerns and improve overall well-being Coordinate with interdisciplinary teams to ensure resident-centered care and promote positive outcomes Facilitate group activities and therapies to enhance socialization and engagement Maintain accurate records and documentation of resident interactions and care plans Collaborate with community resources and outside agencies to access additional support services for residents Requirements Strong communication and interpersonal skills Ability to work effectively in a fast-paced environment and prioritize multiple tasks Commitment to providing high-quality care and services to residents.
