Southwest General Home Health – Licensed Practical Nurse (Home Health) Mon‑Fri 8:30 AM – 5:00 PM Benefits Competitive base hourly rate with an uncapped bonus structure Guaranteed base hourly rate Mileage reimbursement Medical, dental, and vision insurance with flexible options Paid time off 401(k) participation Company‑paid life insurance Employee Assistance Program Qualifications & Attributes Licensed Practical Nurse with current license in the state of employment Minimum 1 year of experience as an LPN in an acute care setting (2 years preferred) Home care experience desired Effective communication and positive impression with patients, families, physicians, and co‑workers Calm, patient, and accommodating under stress Compassionate and caring with patients Knowledgeable in nursing best practices Ability to make appropriate nursing judgments Detail‑oriented and observant Valid driver’s license and auto insurance with your name as a listed driver Major Areas of Responsibility Provides skilled nursing care in a patient’s home as ordered by the attending physician Follows order sets and goals made by RN Observes and monitors patient conditions Notifies RNCM of any changes in patient condition Administers medication as prescribed by the physician Counsels patient and family on disease/injury processes and management Incorporates patient and family in development of plan of care Teaches patients preventive measures to reduce re‑hospitalizations Listens to patient and family members Completes all clinical documentation in accordance with agency protocol and Medicare/Federal guidelines, documenting all aspects of treatment, assessments, and patient care Essential Functions Drives to patient’s primary location per scheduled visit Daily attendance at assigned visit locations Documents all aspects of subsequent visits within 24 hours to meet Medicare guidelines Completes and submits all required documentation within specified company requirements Knowledgeable with providing skilled care within the scope of practice for a Licensed Practical Nurse Fulfills LPN visits in assigned geographic location per patient need within federal and state specifications We are an Equal Opportunity Employer. #J-18808-Ljbffr
