SUMMARY:
Laukoa is seeking a Case Manager to support to the State Surgeons Office for the New Yord Army National Guard (ARNG). This role helps maintain accurate and accessible medical and dental records, assists service members with obtaining and managing their records, and provides administrative support for medical readiness programs and events. The Case Management Assistant will work closely with Soldiers, medical personnel, and other offices to organize and process medical documentation, research information, respond to inquiries, and support Soldier Readiness Processing (SRP) and Medical Readiness Weekend (MRW) activities. This position is well suited for an organized, detail-oriented professional who enjoys helping others and working in a mission-focused environment.


This is a full-time, contract position with a MondayFriday schedule and occasional weekend and in-state travel throughout New York.

• Hourly Rate: $33.83 an hour plus $5.42 H&W applied to the first 40 hours worked /PTO each week
• REQUIRED: Acquiring CAC and passing NACI background check

DUTIES AND RESPONSIBILITIES:

• Responsible for tracking medical status of ALL assigned Soldiers.
• Coordinate Soldier treatment plans with appropriate medical facilities.
• Submit case processing, medical documentation, and profile concerns to military/contacted provider.
• Travel to and attend MRE's events as determined by the needs of the state and the individual units.
• Manage Soldier's care from the identification of the condition to when the Soldier is either:
  • Considered medically ready.
  • Administratively separated from the military.
• Ensure all medical documentation received from the Soldier and/or their medical team is received and placed in appropriated electronic record.
• Maintain Soldier medical records in accordance with Army Regulation (AR) 40-66, Medical Record Administration and Healthcare Documentation.
• Educate Soldiers on resources available to improve medical and dental readiness.
• Maintain Health Insurance Portability and Accountability Act of 1996 (HIPAA) standards and guidelines.
• Review and track health assessment lab results and contact SM and educate on lifestyle modifications.
• Update MODS as required on the following: immunizations, designation of pregnancy, eye exams, and medical warning tags.
• Track medical appointments by communication with unit to ensure Soldier attendance and submission of follow up documentation.
• Report Medical Readiness to the Commander or his/her designated representative.
• Prepare cases for and participate in monthly review of all open case files with Lead Case Manager/DSS.
• Proficient in Microsoft Word, Excel, PowerPoint and Outlook, and proficient with the use of the following; Module of the Medical Operational Data System (MODS) modules: Medical Non-Deployable (MND), Health Readiness Record (HRR), DENCLASS, Medical Data Protection System (MEDPROS), Line of Duty (LOD) and DOJPAAS 9

QUALIFICATIONS:

• Pre-employment background check mandatory.
• Acquiring CAC and passing NACI background check required
• Must have reliable transportation
• Contract employees will be required to travel to locations throughout New York State to meet with service members several times a year.
• Must have computer experience. Knowledge of Microsoft programs such as Excel, Adobe PDF and Word is a must
• Bachelo0rs Degree in Nursing as a Registered Nurse, Masters' of Science in Nursing or Advanced Practical Nurse or, three years of military case management experience as an assistant Case Manager or two years as military Dental Case Manager

Desirable Skills:

• Experience in military, especially Army National Guard, or veterans populations
• Schedule:
  • 8 hour shift
  • Monday to Friday
• Occasional weekend travel in NY
