Registered Nurse (RN) – Home Health
Location: Cullman, Blount & Marshall Counties, AL
Schedule: Monday–Friday | 8:00 AM – 5:00 PM
On-Call: None
We're seeking a compassionate and experienced Registered Nurse (RN) to provide skilled home health care to patients throughout Cullman, Blount, and Marshall Counties. This position is ideal for an RN with home health experience who enjoys working independently while making a meaningful impact in patients' lives.
Position Highlights
- Monday–Friday, 8:00 AM – 5:00 PM
- No on-call responsibilities
- Mileage reimbursement: $0.43 per mile
- Coverage Area: Cullman, Blount, and Marshall Counties
- Productivity Requirement: 30 units per week
- Hours are not guaranteed
Key Responsibilities
- Conduct comprehensive patient assessments and develop individualized care plans.
- Provide skilled nursing care to patients in their homes while promoting health maintenance and disease prevention.
- Monitor patient progress and update care plans based on changing clinical needs.
- Coordinate care with physicians, therapists, social workers, and other healthcare professionals.
- Manage physician orders, medications, and treatments while ensuring compliance with agency policies and regulatory standards.
- Educate patients and caregivers on medications, disease management, and home health services.
- Maintain accurate and timely clinical documentation using agency systems.
- Supervise Licensed Practical Nurses (LPNs) and other support staff as appropriate.
- Coordinate appointments, care transitions, and communication with hospitals and discharge planners.
- Support infection prevention practices and maintain a safe care environment.
- Participate in staff education and training related to health and safety initiatives.
Requirements
- Active Alabama RN License or Compact RN License
- Current, unrestricted Registered Nurse license
- Strong clinical assessment and critical thinking skills
- Excellent communication and case management abilities
- Reliable transportation and ability to travel throughout the assigned territory
