divh2Family Medicine Physician/h2pThis position will float between the family medicine sites of Carolina Health Centers, Inc. in the following counties in South Carolina: Abbeville, Greenwood, Laurens, McCormick, Saluda./ppAs a key member of our Carolina Health Centers, Inc. team, the physician will play a vital role in establishing and overseeing a comprehensive, community-based primary care program. The primary objective of this role is to enhance access to high-quality primary care services, focusing on family-centered care./ppThe physician will be responsible for designing and implementing an integrated delivery model that prioritizes prevention, health maintenance, health education, and evidence-based chronic disease management. By collaborating with a multidisciplinary team, the physician will contribute to improving health outcomes and promoting wellness within the community./ppThe ideal candidate will have a deep commitment to delivering patient-centered care and building strong relationships with patients and their families. Strong communication and interpersonal skills are essential for effective collaboration with colleagues and engaging with diverse patient populations.
