Minimum Requirements

• Board Certified/Board Eligible: Board Certified in Emergency Medicine (ABEM Required); completion of an ACGME-accredited Emergency Medicine Residency
• Certifications: BLS, ACLS, and PALS certifications (or ability to obtain); Indiana Controlled Substance Registration; DEA with Indiana address and schedules 2, 2N, 3, 3N, and 4
• Licensure: MD or DO; active, unrestricted Indiana Medical License
