Nurse Case Manager

Adecco Healthcare & Life Sciences is hiring a Nurse Case Manager in Chicago, Illinois. This role is hybrid, Monday through Friday, 6:45 am to 3:30 pm, with office work Tuesday‑Thursday and remote work Monday and Friday. The position is a one‑year contract with possibility for extension. On‑call rotation follows completion of training. Pay ranges from $40.00 to $44.00 per hour. Responsibilities Coordinate and manage the care of patients with complex medical conditions or disabilities. Work closely with patients, healthcare providers, and insurance companies to ensure that patients receive appropriate care and support. Assess patients' medical, psychological, and social needs. Develop individualized care plans in collaboration with healthcare providers and patients. Coordinate healthcare services, including treatments, therapies, and consultations. Monitor patient progress and adjust care plans as needed. Provide education and support to patients and their families. Advocate for patients' needs and rights within the healthcare system. Communicate with providers, insurers, and stakeholders to ensure coordinated care. Evaluate the effectiveness of care plans and recommend improvements. Document patient assessments, care plans, and interactions. Keep abreast of healthcare regulations, policies, and best practices related to case management. Qualifications High school diploma or GED. Active and in good standing Registered Nurse in Illinois. 2+ years of nursing experience. Occupational health experience preferred. Knowledge of case management for workers' compensation, disability, limited duty, safety, industrial hygiene, healthcare plans, regulatory medical requirements, and external resources.
