Case Manager for SUD Residential Treatment

$18 - $20 per hour

Job Details Job Location: TCN Christopher House - Xenia, OH 45385 Salary Range: $18.00 - $20.00 Hourly Travel Percentage: Up to 25% Job Shift: Day Position Summary This position is responsible for linkage services in support of clients with substance use and/or dual diagnosis disorders currently residing in Christopher House or any future residential treatment facilities. The role works closely with other residential and TCN outpatient providers to ensure needed services are provided to the clients in residential treatment. Services provided are focused on promoting clients’ ability to succeed in residential treatment, as well as in the community once residential treatment is completed. The position assists and supports the clients in identifying and gaining access to needed medical, social, and other essential services. It also supports the Associate Director of SUD Residential Services with property upkeep by working closely with TCN’s Maintenance, MIS, and Financial departments. The position may also cover SUD Outpatient groups as needed. Key Responsibilities Provides individual case management, group counseling, and/or individual counseling under clinical supervision while maintaining a residential environment that promotes positive resident growth and achievement of treatment goals, meeting agency quality requirements: Completes intakes and orientation for new clients in residential treatment. Assists in designing and participating in resident activities. Assists in identifying resident treatment needs and providing appropriate interventions as directed by supervisor. Facilitates educational/therapeutic groups and other therapeutic activities as needed. Reinforces resident orientation to house rules, safety procedures, and household environment. Utilizes the residential plans of care for each resident. Monitors the provision of self‑assisted medication. Monitors each client’s general health status. Ensures facilities are pleasant and meet cleanliness standards of the Department of Behavioral Health, TCN, and county health department. Respects and protects clients’ rights. Completes all safety procedures and drills in strict accordance with agency schedules and standards. Transports clients as needed using company vehicles and/or personal vehicles pending approval from TCN’s drive‑check agency. Accompanies clients to various locations, ensuring continuous monitoring throughout their activities (stores, appointments, waiting rooms, support meetings, etc.). Thoroughly reads, comprehends, and consistently enforces all client rules without exception, escalating any violations to supervisor. Reports daily activities and other pertinent information. Documents client progress and activities in residential charts and daily shift reports. Assists in the preparation of facility summary reports. Engages in ongoing communication with community support personnel, supervisors, and other clinicians to relay information and updates. Maintains 100% accuracy in documentation and summary reports. Completes documentation according to TCN standards. Provides only those services authorized by Utilization Management and within authorized frequency and duration. Defines service plans and progress notes with client input and signature when clinically appropriate. Completes progress notes at the end of each session and submits all documentation within 24 hours. Develops and maintains quality, accurate clinical charts for all clients served. Handles crisis situations calmly and effectively. Mediates conflict among residents, promoting a respectful and peaceful living environment. Maintains open communication with clients, staff, and stakeholders. Demonstrates alertness, approachability, availability, and readiness. Maintains agency‑required annual training: client rights, grievances, safety, AEGIS (de‑escalation), infection control, and others required by the Department of Behavioral Health. Develops positive collaboration with other social service agencies, maintaining knowledge and familiarity with community referral sources. Serves as liaison between the criminal justice system and treatment services in jails. Performs all other residential treatment and/or clinical duties as assigned. Qualifications Education: Associate degree in human/social service field. Experience: One year in alcohol and substance abuse residential treatment preferred. Certification: CDCA or above required, SWA or above preferred; CPR; First Aid; AEGIS (De‑escalation) training; Medication Monitoring. Other: Valid Ohio driver’s license; proof of automobile insurance; meets ODADAS licensure and certification requirements; demonstrated competencies for working with persons with substance use and/or mental health disorders. Knowledge, Skills, & Abilities Knowledge of safety rules and procedures, agency policies and procedures, local resources, daily living tasks and skills, documentation requirements, crisis management, empathy and compassion, and ethical conduct. Skills: Capacity to perform duties effectively under emotional stress and conflict; proficiency in basic computer and office equipment for documentation and communication; client‑focused approach with active listening, reading comprehension, critical thinking, and leadership; excellent interpersonal skills including directing and demonstrating daily living activities (cooking, cleaning, laundry); basic household appliance and tool operation. Abilities: Willingness and physical capability to engage in essential activities (relocating individuals, shopping, instructing daily living skills as necessary); lifting up to 25 pounds; standing, bending, stooping; writing legibly or using computer; clear vision, hearing, reading, and communication; recognition of unusual or threatening conditions; patience and understanding with individuals at different stages of recovery. #J-18808-Ljbffr
