Pay Range: $40.07 - $48.10 per hour, depending on prior experience.

• Graduation from an accredited nursing program; a Bachelor's degree is preferred.
• Current RN license in the state of Colorado.
• A minimum of two years of nursing experience, including long-term care, dementia care, or acute care; home care experience is preferred.
• CPR certification.

The Registered Nurse (RN) will be responsible for:

• Managing cases, coordinating, supervising, and implementing professional and supportive services for clients, utilizing nursing theory and processes in line with the Nurse Practice Act.
• Reviewing client referral information and responding to inquiries as necessary.
• Conducting client assessments according to agency standards to ensure effective home care services.
• Independently directing, planning, and initiating appropriate actions in home care situations.
• Performing duties in accordance with agency policies, procedures, and professional standards.
• Completing additional tasks as assigned by the Director of Clinical Services.

Our organization is a Medicare/Medicaid-certified home health care agency located in Montrose, CO. We have a dedicated team of nurses, rehabilitation therapists, and home health aides focused on delivering personalized healthcare with compassion and empathy. Our expertise lies in caring for older adults, providing in-home health services across Delta, Montrose, and Ouray counties.

• 2 years of nursing experience, including a mix of long-term care, dementia care, or acute care.

• Experience in home care.

• Bachelor's degree in Nursing or a related field.

• CPR/First Aid Certification
• Registered Nurse License
