Tops Markets is seeking a Hybrid Clinical & Community Pharmacist to join our Pharmacy team in the Hudson Valley. This unique position is designed for a pharmacist who wants to remain actively involved in community pharmacy practice while also having dedicated time to provide and develop clinical pharmacy services. The Hybrid Clinical & Community Pharmacist will divide their time between community pharmacy practice and clinical pharmacy services, supporting multiple pharmacy locations while helping expand patient-care initiatives across the organization. The role provides an opportunity to practice at the top of the pharmacist license through a combination of direct patient care, medication management, immunizations, clinical interventions, provider collaboration, and traditional community pharmacy practice.
Hybrid Practice Model
Community Pharmacy Practice
Provide pharmacist coverage at designated pharmacy locations.
Review, verify, and dispense prescription medications.
Counsel patients regarding prescription and nonprescription medications.
Oversee pharmacy workflow and support pharmacy technicians and interns.
Maintain compliance with federal, state, and company requirements.
Support immunizations and other patient-care services.
Assist pharmacy teams with operational and patient-care needs.
Clinical Pharmacy Services
Provide dedicated clinical pharmacy services across assigned locations.
Perform comprehensive medication reviews and other medication therapy management services.
Identify medication-related problems and opportunities to optimize therapy.
Conduct targeted patient outreach related to adherence, medication gaps, and clinical opportunities.
Identify and close immunization and preventive-care gaps.
Support disease-state management initiatives and patient education.
Collaborate with prescribers and other healthcare professionals regarding medication therapy and patient-care opportunities.
Participate in community health events, immunization clinics, screenings, and other outreach programs.

Essential Duties and Responsibilities
Follow all Federal, State, and Local regulations as well as company policies regarding Pharmacy operations, safety, and sanitation
Process customer orders while providing attentive and courteous customer service
Required to perform the final check on all medications before dispensing to the customer
Receive telephone prescriptions and authorizations from prescribers offices
Consult with patients about prescribed medications and other health issues and recommend over-the-counter medications
Achievement of financial goals such as sales, gross profit maximization as well as minimization of shrink and supply expense
Adhere to company policy through proper processing, preparation, and packaging of prescriptions
Train and develop pharmacy interns and technicians
Complete all necessary paperwork relating to Pharmacy Department
Maintain security of all controlled substances
Maintain adequate supplies, place orders when needed
Process orders to include filling prescriptions and accepting payment
Minimize shrink through proper handling and storing of products
Assist pharmacy teams in improving workflow, patient engagement, and delivery of clinical pharmacy services.
Participate in the development and implementation of new pharmacy programs and services.
Travel among assigned Tops' pharmacy locations as needed.
Deliver preventive and clinical healthcare services, including immunizations, medication management, patient education, and other approved pharmacy services.
Minimum Qualifications
Must be at least 18 years of age
State Licensed Pharmacist
Ability to travel to various stores as need to work as the licensed pharmacist
Education and Experience
Required education to be a State Licensed Pharmacist
Ability to read, write, and perform basic math functions
Possess basic computer skills
Possess strong communication skills
