RN Case Manager

Brief Description Our RN Case Managers are responsible for coordinating and managing the care of many of our high‑risk patients, such as those with multiple chronic conditions. Duties include meeting with patients to manage their medical conditions, providing education, developing a care plan, appropriate follow‑up and coordinating care with other clinical providers. As an essential member of our integrative care team, you will collaborate with multi‑specialty colleagues to ensure that our patients are provided the highest quality of care.
