In this role, you will be responsible for managing the assessment process, ensuring accurate and timely completion of MDS assessments, and coordinating with the interdisciplinary team to develop individualized care plans that meet resident needs and regulatory requirements./ppKey Responsibilities:/pulliOversee and complete the Minimum Data Set (MDS) process in compliance with federal and state guidelines./liliCollaborate with nursing, therapy, and other departments to ensure accurate documentation and assessment./liliMonitor and audit records for accuracy and completeness./liliParticipate in care plan meetings and ensure care plans are updated as needed./liliStay current with regulatory changes affecting MDS and reimbursement processes./li/ulpQualifications:/pulliCurrent LPN or RN license in Illinois (required)./liliPrevious experience as an MDS Coordinator in a skilled nursing facility./liliStrong knowledge of RAI process, PPS, and Medicare guidelines./liliExcellent communication, organization, and time management skills./liliProficiency with electronic medical record systems./li/ulpWe offer a supportive work environment, competitive compensation, and benefits.
