Job Title: Case Manager | RN - Registered Nurse Location: Baltimore, MD Setting: Hospital Pay Range: 1617.28 - 1698.14 Schedule: 08:30 - 17:00 Contract Length: 13 weeks Candidate Type: Traveler Requirements: Active RN License; 2 years RN experience; BLS; Current in-patient Case Management experience; post-acute facility referral platforms such as NaviHealth or AllScripts

Responsibilities:

• Facilitate discharge planning and coordinate patient care for pediatric units
• Assist in managing in-patient case loads across multiple pediatric units including NICU, PICU/PCICU, Onc, and others as needed
• Support bedside care with experience in case management and referrals to post-acute facilities
• Collaborate with multidisciplinary teams to ensure smooth patient transitions and discharge processes
