They are currently seeking a compassionate Care Manager RN who brings a servant-leadership mindset to their work. This role is centered on care coordination for medically complex patients and their families in a field-based environment, throughout the greater Austin metro.

No two days look the same, your office might be your car, a patient's home, or a provider visit. It's ideal for someone who enjoys autonomy, can confidently navigate complex situations, and values building meaningful, long-term patient relationships. If you prefer a predictable desk role, this likely isn't it, but if you're energized by being out in the field and advocating in real time, it could be a strong fit.

What's being offered:

• Full-time, permanent position; Mon-Fri with full benefits and PTO starting at the 60 day mark
• Salary range: $80,000–$92,000/year (based on experience)
• Mileage reimbursement + monthly tech stipend
