Registered Nurse - Home Health

Job Description

Job Summary:

The Home Health RN - Full Time provides individualized, goal-oriented nursing care to patients in their homes using the nursing process and in accordance with physician orders. In cases where the RN is the lead discipline, the role also functions as a case manager to coordinate care and support patient outcomes.

Responsibilities:

• Assess patients, develop nursing care plans, and implement interventions based on individualized goals.
• Provide skilled nursing care in accordance with physician orders and current clinical standards.
• Monitor patient status, evaluate outcomes, and document progress using accurate, timely records.
• Coordinate care as the lead discipline case manager, including communication with the care team and relevant stakeholders.
• Educate patients and families on care plans, self-management, and recommended health practices.
• Ensure continuity of care by coordinating referrals, follow-ups, and appropriate services.

Qualifications:

• Hold an active Registered Nurse (RN) license.
• Demonstrate strong clinical assessment and patient care planning skills.
• Apply the nursing process effectively, including documentation of assessments, plans, interventions, and outcomes.
• Possess case management experience or the ability to assume lead discipline responsibilities when required.
• Maintain current knowledge of home health practices, safety protocols, and evidence-based care.
• Communicate professionally with patients, families, and interdisciplinary teams.
