Occupational Therapist – Home Health Case ManagerSeeking an experienced Occupational Therapist Case Manager to support Home Health services in the North Metro territory. This is a full-time, weekday-only contract with no on-call or weekend requirements—solid structure, consistent workflow, and a traditional home health model that still works.The OT Case Manager will independently manage a home health caseload, providing occupational therapy services, coordinating patient care, and ensuring accurate documentation and compliance with home health regulations. The role requires strong clinical judgment, organization, and patient-centered care in the home setting.Manage and coordinate a home health OT caseloadPerform initial evaluations, reassessments, and discharge planningDevelop and implement individualized OT care plansProvide therapeutic interventions focused on ADLs, safety, and functional independenceCollaborate with interdisciplinary home health team membersEducate patients and caregivers on treatment plans and home safetyEnsure timely, accurate clinical documentationComply with regulatory, quality, and productivity standardsAdult and geriatric home health patientsVarying diagnoses requiring functional, safety, and independence-focused OT careActive Occupational Therapist license (WI or compact, as applicable)Home Health OT experience strongly preferredAbility to work independently in patient homesStrong communication and time-management skillsMust be willing to float to surrounding territories as neededLeadership and operational support availableTraditional case management model with predictable schedulingNo weekends, no holidays, no on-callStrong hourly rate for standard home healthStable weekday scheduleIdeal for OTs who value autonomy and routine
