Licensed Professional Counselor or Licensed Marriage and Family Therapist Be among the first 25 applicants Position Summary Eventus WholeHealth delivers an integrated model of care to adults who reside in skilled nursing and assisted living facilities through a network of healthcare providers including physicians, nurse practitioners, physician assistants, medical assistants and in‑house support staff. We are seeking a Therapeutic Behavioral Specialist to join our team. You will provide recovery‑oriented support and case management services to residents and their families, with opportunities for a future transition to counseling services. We address a wide range of problems from anxiety and depression to severe and persistent mental illness to issues around death and dying. As part of a multidisciplinary team, our clinicians have the chance to make a big difference in the quality of life for these residents. Essential Duties and Responsibilities Provide therapeutic behavioral services to clients, consulting with a licensed clinician to meet individual needs. Create service plans and coordinate care, including behavioral health and development of a treatment plan. Offer linkage, interventions, treatment options, restoration of daily life skills and crisis prevention services. Provide services to individual clients or groups of clients. Train support systems, including direct staff, on client triggers, de‑escalation techniques and interventions as outlined by the licensed clinician. Promote a positive and cooperative relationship with all outside contacts. Meet with resident care coordinators or other staff as designated by the facility to review current resident and facility care needs and communicate findings or concerns upon arrival and departure. Maintain open and positive communication with facility staff and administration; provide verbal and/or written instruction or feedback regarding treatment plans and other pertinent caregiver information. Adhere to Eventus WholeHealth standards, policies and procedures, and all legal and regulatory requirements, including professional ethical standards. Conduct oneself in a professional manner at all times. Maintain patient confidentiality at all times, including appropriate use of cell phone, email, text messaging, patient records and EMR. Be knowledgeable of HIPAA rules and regulations. Consult with the clinical supervisor when facing an unfamiliar resident, family or facility request or recommendation regarding the care of a resident. Qualifications / Education Licensed Professional Counselors and Licensed Marriage and Family Therapists – Master’s degree in either counseling or marriage and family therapy, with eligibility to practice in the state. Knowledge Population‑specific interventions and delivery of care following treatment plans. Rules and regulations of bodies governing behavioral health practice. Organization’s policies and procedures. Skills Required ing and analyzing physiological, socioeconomic, behavioral, and emotional patient data. Providing training in social and emotional well‑being, stress management, anger management and overall life skills. Accurately evaluating patient problems and providing appropriate advice, intervention or referral. Written and verbal communication. High degree of initiative, judgment and discretion. Establishing and maintaining effective working relationships. Abilities React calmly and effectively in emergency situations. Work collaboratively with all members of the health care team. Evaluate and make recommendations for continuous quality improvement. Handle confidential and sensitive information.
