Description

• Conduct thorough evaluations of patients in either the inpatient or outpatient settings, crafting tailored treatment strategies
• Implement physical therapy interventions aimed at enhancing mobility, strength, balance, and overall functionality
• Work alongside a dedicated team of nurses, physicians, case managers, and other rehabilitation specialists to drive optimal patient outcomes
• Inform and educate patients and their families about therapeutic objectives, planning for discharge, and home exercise routines
• Maintain comprehensive documentation of evaluations, treatments, and patient progress in alignment with regulatory and facility standards
• Engage actively in interdisciplinary conferences and care team discussions to foster coordinated care efforts

Education

• Doctorate in Physical Therapy (DPT) or equivalent degree from a recognized program

Certifications

• State licensure as a Physical Therapist
• Preferred: CPR certification
