Admissions Liaison

As an Admissions Liaison at Goldwater Care, you will serve as a primary point of contact for prospective residents, families, representatives, referral sources, and community partners, providing a welcoming and professional experience throughout the inquiry, assessment, intake, and admission process. Essential Duties and Responsibilities: Serve as a primary point of contact for prospective residents, residents, families, representatives, referral sources, and community partners, providing a welcoming and professional experience throughout the inquiry, assessment, intake, and admission process. Conduct bedside assessments and referrals at hospitals, healthcare facilities, and other locations in collaboration with prospective residents, representatives, families, case managers, social workers, and other healthcare professionals to evaluate appropriateness for admission. Gather and verify necessary demographic, financial, medical, and referral information to support accurate, timely, and compliant admissions. Coordinate and facilitate the admission process, including communicating with prospective residents, families, referral sources, and facility staff regarding admission requirements, placement, and available services. Requirements: Must possess a minimum of 2 years experience in a healthcare setting. Ability to communicate effectively, orally and in writing. Strong organizational and time management skills. Ability to maintain confidentiality and handle sensitive information.
