In-Home Traveler Nurse Practitioner or Physician Assistant

Signify Health is seeking a part-time In-Home Traveler Nurse Practitioner or Physician Assistant to perform comprehensive health evaluations for adults in their homes. You will review medical histories, perform physical exams, assess social determinants of health, and close care gaps for value-based care programs. Using Signify’s mobile technology and analytics, you’ll document findings, coordinate with health plans and providers, and recommend follow-up care. This role involves regional travel, autonomous practice within your license, and a strong focus on quality, safety, and patient-centered care.
