This role works directly within long-term care and senior living communities, supporting residents, partnering with facility staff, and helping deliver proactive healthcare services through MedStreamline's innovative care model.Position SummaryThe Onsite Licensed Practical Nurse (LPN) / Licensed Vocational Nurse (LVN) is responsible for providing onsite clinical support services within assigned long-term care and senior care facilities. This role serves as a key clinical representative of MedStreamline and works directly with residents, facility staff, and MedStreamline leadership to support patient assessments, documentation, device utilization, and care coordination activities.The ideal candidate is patient-focused, self-motivated, adaptable, and comfortable working independently across multiple facilities within an assigned territory while maintaining a high standard of professionalism, compliance, and service excellence.What You'll DoPatient Care & Clinical Support• Perform resident assessments and collect complete vital signs using MedStreamline's FDA-approved monitoring devices.• Accurately document patient information, observations, and assessment results according to company standards.• Support proactive resident monitoring and care coordination initiatives.• Identify and escalate clinical concerns, changes in condition, or potential risks to appropriate facility personnel and MedStreamline leadership.• Maintain professional and compassionate interactions with residents, families, caregivers, and facility staff.• Promote resident comfort, dignity, and patient-centered care throughout all interactions.Facility & Service Delivery Support• Travel daily to assigned long-term care, skilled nursing, and senior living facilities within a designated territory.• Serve as an onsite MedStreamline representative and trusted resource for facility staff.• Support implementation and utilization of MedStreamline services and technology within partner facilities.• Assist with troubleshooting and basic operational inspections of MedStreamline equipment during facility visits.• Report on equipment concerns, maintenance needs, and operational issues to appropriate internal departments.• Provide basic device education and guidance to facility staff when appropriate.• Support facility relationship development through consistent professionalism and exceptional customer service.Documentation & Compliance• Maintain accurate, timely, and complete documentation in accordance with MedStreamline policies and procedures.• Comply with HIPAA, state nursing regulations, infection control standards, and all applicable healthcare requirements.• Follow company protocols regarding patient privacy, data security, and confidentiality.• Participate in audits, compliance reviews, and quality assurance initiatives as required.• Maintain all required licenses, certifications, and credentialing requirements.Collaboration & Communication• Partner closely with facility leadership, nursing staff, Account Managers, and Clinical Operations leadership.• Communicate clinical observations, operational concerns, and service opportunities effectively.• Participate in staff meetings, educational programs, training sessions, and company initiatives.• Assist with identifying workflow improvements that enhance patient care and operational efficiency.
