About the Role:

We are looking for a reliable, self-disciplined individual who can work independently with minimal supervision, is highly responsible, and maintains professional boundaries. The Community Health Worker (or Promotor at ¡Vamos! Health) plays a crucial role in promoting health and wellness among the patients we serve. This position works closely with community members to provide culturally competent health education, support, and navigation services that connect patients to the resources they need — from healthcare and social services to community-based programs. The CHW/Promotor acts as a trusted liaison between the healthcare provider and the community, guiding patients through the healthcare system, facilitating access to services, and addressing the social determinants of health that impact their well-being. Above all, the CHW/Promotor is committed to delivering an outstanding patient experience — building trust, showing up with empathy, and ensuring every patient feels supported, heard, and cared for at every step of their health journey.

This role will report to the Community Health Lead.

Essential Duties and Responsibilities:

New Patient Support

• Coordinate patient onboarding and account creation
• Coordinate and schedule appointments
• Provide assistance and/or referrals to help patients access other essential support services
• Help patients understand medical instructions and connect with available resources
• Serve as a liaison between patients and providers to ensure clear communication and understanding
• Conduct SDOH and Needs Assessment surveys

Building Relationships

• Establish and maintain trusting, respectful relationships with patients
• Adapt communication style to resonate with the community served
• Foster a respectful and inclusive environment that values diversity
• Serve as the key point of contact for patient needs
• Provide emotional support and encouragement to patients, fostering a sense of trust and safety
• Educate patients about available health services and community resources

Healthcare Navigation

• Maintain a strong understanding of the overall healthcare system, community resources, and ¡Vamos! Health services, and how they work together
• Provide patients with relevant resources and education
• Utilize digital platforms to distribute educational materials, videos, and resources to patients
• Update patient resource plans based on changing needs and circumstances
• Organize and lead virtual and in-person health workshops, webinars, and support groups
• Identify and address barriers to care promptly, developing solutions to improve access and quality of services

Data Collection & Reporting

• Accurately enter and update patient information, medical histories, and visit notes in designated systems
• Document patient interactions accurately and confidentially in accordance with HIPAA guidelines
• Compile and report data on patient health trends, needs, and outcomes
• Participate in regular team meetings
• Provide ongoing insights and feedback to the Patient Care Lead regarding patient experiences or concerns

Ongoing Patient Engagement

• Engage in care planning and care plan management
• Follow up on referrals to specialists and outside organizations by confirming appointment dates, ensuring patients have transportation, sending appointment reminders, and documenting the process as required
• Resolve barriers to accessing resources
• Manage ongoing communication with patients regarding membership, medical, and community-based needs
• Complete assigned certifications, training, and continuing education to strengthen patient engagement and health condition management

Other Duties

• Participate in regular meetings and training to stay informed on best practices and developments in community health
• Attend training, workshops, and conferences to stay current on emerging trends in the field
• Collaborate with healthcare providers, clinic staff, and fellow CHWs/Promotores to ensure coordinated care and services for patients
• Participate in interdisciplinary team meetings to discuss patient cases and develop comprehensive care plans
• Maintain flexibility to work occasional evening and weekend hours as needed
• Perform other duties as reasonably assigned

Qualifications

• Bilingual proficiency in English and Spanish (written and spoken)
• Minimum of 3 years of experience in community health, community health education, social services, outreach, or a related field
• Knowledge of health promotion, disease prevention, and community resources
• Strong interpersonal and communication skills
• Ability to work independently as well as collaboratively within a team
• Strong critical-thinking and problem-solving skills
• Deep understanding of and sensitivity to cultural nuances
• Appreciation for and understanding of diverse cultures and communities
• Strong organizational skills and attention to detail
• Certification or training in community health work, public health, or a related field preferred
